Our Care Process

Understanding the Unique Needs

In order for us to ensure that your loved one is properly cared for, we have an in-depth consultation to understand the unique needs of your loved one. No patient is identical and we understand that they need personalized in-home care.

Creating the Optimal Care Plan

After our consultation process, we begin planning the best way to care for your loved ones. We dissect every aspect of their life and needs in order to effectively create a personalized care routine for them. After the plan is formulated, we will review that plan with you. That way, we ensure that your input is taken to the highest level of consideration and so that way every aspect is covered effectively.

Put the Plan Into Effect

After the plan is reviewed and vetted, it's time to implement this personalized in-home care plan. We document the progress of both your loved one and the plan's effectiveness in respect to the quality of care needed.

Change the Plan for the Clients Benefit

After careful documentation and strict enforcement of the plan in place, we review the plan's effectiveness with you. We keep you informed at every stage so you may have peace of mind knowing that we are doing the utmost for the health and well-being of your loved one in our care. After review and revision, we implement the newly formulated in-home care plan. This process happens on a quarterly basis to ensure that all previous conditions are being effectively cared for and all emerging complications are addressed.

  • Where is your main office?

    We are based out of Springfield MA. Due to COVID all consultations and additional communications aren't conducted in person for the safety of our staff. That way, we mitigate any potential and unnecessary exposure. In addition, this enables us to continue to operate and service those that need personalized in-home care.

  • What are your business hours?

    Our office hours are Monday - Friday, 8:00 am - 5:00 pm, and appointment only on Saturdays and Sundays. Current patients can still contact us after hours if needed. If it is an emergency, please contact your local emergency services immediately.

  • Do you offer care for out-of-state residents?

    At this time, Elim Health does offer services to out-of-state residents that are within our service area. We currently operate within a 50-mile radius of Springfield, Massachusetts. 

  • Do you offer Telehealth visits?

    No, Elim Health LLC does not currently offer telehealth visits in order to diagnose issues. Call our office line or email us regarding the best way to assist you.

  • What types of insurance do you accept?

    Currently, we do not accept any forms of insurance. We plan on expanding our payment options in the future.

  • How do I pay for in-home care services?

    We currently only accept "Private-pay" as the only means to pay for our provided in-home care services. 

  • Do you subcontract your services?

    No, Elim Health does not subcontract its services to other Agencies. Elim Health hires and services clientele ourselves, the way it should be.

  • How do you match up caregivers and clients?

    At Elim Health, we ensure the compatibility of our clients and caregivers based upon your loved one's needs,  our healthcare professional's credentials, and your location.
